I have a DViCO HDTV fusion 3 (with QAM) I think all of their cards since
have been QAM, it works well you just have to build the drivers in the
kernel.
I also have an ATI HDTV Wonder, it also has QAM support. Both cards
work with MythTV, I just haven't spent the time to make it all work
nicely together yet.
avsforum.com has been an awesome resource for me in the past for any HD
related projects in windows or linux.

The ATI card I bought somewhere when they were having a special sale.
Most cards are decently supported these days, and since they all they
have to do is tune the stream you can dd the mpeg2 right from the
device. Just make sure to trim the edges a bit. ;)
