I would assume that an ISP would do it like this(all numbers fictitious) :
they are allocated 100 GB
their average customer uses 5 MB
They have 20 customers (20 * 5 = 100)
Usually Several customers use 1 or 2 MB and several use 8 to 10.
This all balances out on a standard basis.
They have alloted in to their total cost a figure for several to use 9 and
push the total to say 125.
Then they allow each user 10 (20 *10 = 200) Knowing that on the law of
averages that will never happen.
Then when you go over the 10, that helps them recoupe the cost of those
months when it hits 125.

Makes sense to me and is how I would do it if I were an ISP
