Yes, you can get  just DSL with no phone line. I have this exact setup (with
Xmission as the ISP). My bill to qwest is $33/mo. Xmission is a separate
bill. For that, it's $19 if you go month to month, or a few bucks less if
you pay 6 or 12 months at a time.

To get the DSL without phone service & with your own ISP, you have to call
Qwest and specifically ask for it. They don't have that option listed on
their site. At least they didn't when I signed up.

I have a 1.5mbit download, 900kbit or so upload (I forget what the exact
upload speed is). I haven't had any reliability problems with the line or
with Xmission, and the speed is generally good.

I didn't have to.

4.  How does that work with the modem?  Do you just rent one, or can you

You can rent one for $5/month, or buy one for $60. I bought mine.
