Steve,

You will have to get a line from Qwest, unless you go with a premium
ISP like Speakeasy.net or the likes.

Xmission is a great local company. Their plans are on top of the line
fees for Qwest (as I understand it).

So you would be paying for the Qwest Line (yes you can get it without
voice) + Xmission ISP.

So in Short:

1) Yes, you can get just a DSL line
2) Depends on the line you pay for but between 384/384 and 1.5/768
3) Qwest, Yes. Xmission, depends.
4) Usually you rent the modem from Qwest (they are cheap but they
work) for $5/month (last I checked). You can buy one at most big box
stores.
